Skilled nursing care at home encompasses a wide array of both health and supportive services. Patients span the spectrum of care, from seniors who need assistance with daily activities in their homes; to post-surgical patients needing assistance with wound care; to the chronically ill who are maintained with skilled nursing care, support services, home modification and equipment.
Home health care is based on an individual's need developed in collaboration with clinical experts, the patient and the family. The amount of care required may range from 24-hours-a-day to monthly visits from skilled nurses, therapists, social workers and home health aides.
RNs and LPNs provide nursing care and education for patients with acute and chronic conditions. As need dictates, the services of physical, occupational, or speech therapists, medical social workers, and/or home health aides can be added.
